Chase joined Hamot Health Foundation team in October 2025 as the Community Outreach Coordinator. Chase plays a central role in building partnerships, expanding access to resources, and strengthening trust between marginalized community members and healthcare systems. He can often be found inside primary care offices, the UPMC Health Plan Neighborhood Center, Mental Health Association of Northwest PA, Erie City Mission, and other partner organizations, serving as a critical resource for patients, providers, and staff.
Passionate about food access and health equity, Chase integrates nutrition support, healthcare navigation, and social services in his daily work. He not only helps lead HHF’s food rescue efforts, but he also volunteers at the Mercy Center for Women helping prepare meals and teach vulnerable women how to cook.
Known for his creativity and collaborative spirit, Chase is deeply embedded in Erie’s service community. He participates in monthly outreach with Harvest 912, providing foot care, wound support, clothing, and survival supplies in encampments.
He brings a strong blend of operational expertise and community engagement to this role. Chase holds a Bachelor of Science in Strategic Communication from Edinboro University and served as an Emergency Preparedness Specialist for UPMC Hamot from 2021 to 2025. In this position, he coordinated large-scale public health initiatives and managed critical supply systems—including COVID-19 vaccination efforts—with federal, state, and local agencies.
Chase’s commitment, systems knowledge, community presence, and innovative spirit make him an essential member of HHF’s outreach team and a key driver of the program’s impact.
