Street Outreach

RIGHT CARE, RIGHT PLACE, RIGHT TIME

Street Outreach brings health care, vital resources, and personalized guidance to vulnerable neighbors.

Led by a registered nurse and three community health workers, the team meets people where they are to help with:

  • Assessing needs and navigating resources (e.g., food assistance, transportation, health insurance enrollment, debt relief, shelter)
  • Scheduling medical, dental, and mental health/counseling appointments
  • Administering over-the-counter medications and safely storing prescription medications, preventing
    theft, misuse, and misplacement
  • Performing health screenings and vaccinations
  • Partnering to complete applications for safety-net programs, housing, employment, and more
  • Providing education and lifesaving supplies

Street Outreach focuses on helping people meet their basic needs first, bringing high-quality, relationship-based health care, and support to people in need.

Street Outreach meets people where they are - whether that’s a homeless shelter, encampment, community center, or sidewalk. Care adapts around people’s realities, centering equity and dignity.

  • Reduced avoidable emergency department visits and hospitalizations
