Street Outreach

RIGHT CARE, RIGHT PLACE, RIGHT TIME

Street Outreach brings health care, vital resources, and personalized guidance to vulnerable neighbors.

Led by a registered nurse and three community health workers, the team meets people where they are to help with:

  • Assessing needs and navigating resources (e.g., food assistance, transportation, health insurance enrollment, debt relief, shelter)
  • Scheduling medical, dental, and mental health/counseling appointments
  • Administering over-the-counter medications and safely storing prescription medications, preventing
    theft, misuse, and misplacement
  • Performing health screenings and vaccinations
  • Partnering to complete applications for safety-net programs, housing, employment, and more
  • Providing education and lifesaving supplies

Street Outreach focuses on helping people meet their basic needs first, and meets people where they are. From shelters and warming centers, to soup kitchens, and anywhere where care can be provided.
